LaCluyze, University of North Carolina at Chapel Hill, on behalf of the CHASE project, report the discovery of an apparent supernova (mag approximately 18.4 +/- 0.4) on an unfiltered image taken on Nov. 12.32 UT with the 0.41-m 'PROMPT 5' telescope located at Cerro Tololo. The new object is located at R.A. = 6h42m12s.52 +/- 0".1, Decl. = -61o04'27".2 +/- 0".1 (equinox 2000.0), which is about 4".6 west and 12".2 south of the center of the galaxy ESO 122-4. Nothing is visible at this location on an R-band image taken on 1988 with the European Southern Observatory 1-m Schmidt telescope (limiting mag 19.0). Additional approximate unfiltered CCD magnitudes for 2009ll: 2008 Sept. 1.32-2009 Nov. 3.31, [20.5 (thirty-eight 40-s stacked archival images); 2009 Jan. 31.10, [19.0; Oct. 8.36, [18.5; Nov. 8.34, 18.7; 13.11, 18.3 (seven 40-s exposures); 19.10, 18.2 (thirty-one 40-s images).
